Please call our billing office at 615-790-7992 with any questions or concerns regarding your account. The hours for the billing office are Monday through Thursday, 9AM – 5PM.

Self-pay Patients

We welcome self-pay patients and provide a discount for the uninsured. Payment in full is expected at the time of service

Insurance

For patients with insurance, all copayments and balances will be collected at check-in. We will submit the claim to your insurance company, but you will be responsible for any outstanding balance. Please be sure to bring your insurance card to every visit. Please note that the patient is responsible for verifying insurance coverage for all services. Also, if you have an HMO insurance and another doctor is listed on your card as the PCP, the insurance company may deny coverage for your visit; in this circumstance, payment in full will be required at the time of service.
